Why conduct this study?

Endothelial dysfunction is one of the earliest signs of vascular damage in a range of different diseases, including diabetes, hypertension, hypercholesterolaemia and coronary heart disease.1 In the kidney, endothelial dysfunction can result in intraglomerular hypertension and inflammation, and consequently albuminuria.

Angiotensin II can worsen endothelial dysfunction by increasing blood pressure and oxidative stress.2 As a result of their actions on the renin-angiotensin system, the angiotensin II receptor blockers (ARBs) losartan and irbesartan have been shown to exert a protective effect, even in the early stages of renal disease.3 Other ARBs and ACE inhibitors may also exert such a protective effect.

The aim of the TRENDY study was to compare the mid-term effects of MICARDIS® and ramipril on renal plasma flow after stimulation with an inhibitor of endothelium-dependent vasorelaxation (induced by nitrous oxide) in patients with type 2 diabetes and mild-to-moderate hypertension.4
